Chronic Pain Care Plan

Understanding Your Chronic Pain Care Plan

This plan explains how we’ll work together at Silky Oak Chiropractic to manage your chronic pain from a musculoskeletal condition. It outlines the process, highlights the importance of manual therapy, and provides a clear timeline for your care.

  1. What is Chronic Pain?

Chronic pain is pain that persists for longer than 3 months 1. It can be ongoing or intermittent, and it often significantly impacts your daily life. While there may not be a quick fix, there are effective ways to manage your pain and improve your quality of life.

  1. Our Focus: Creating Change and Maintaining Progress

At Silky Oak Chiropractic, we understand that chronic pain requires a different approach than acute pain. Dr. Jan Jones will use a variety of manual therapy techniques to address the underlying causes of your pain and help your body function better. These techniques may include:

  • Spinal and Joint Manipulation: To improve joint mobility and reduce stiffness.
  • Soft Tissue Work: To release tension and improve flexibility in muscles and other soft tissues.
  • Trigger Point Therapy: To address specific areas of muscle tightness and pain.
  1. Your Journey to Long-Term Wellbeing

Your care will focus on two key areas:

  • Creating Change:
  • What to Expect: Manual therapy to address joint restrictions, muscle imbalances, and other factors contributing to your pain. Dr. Jones may also recommend exercises or stretches to support these changes.
  • Your Role: Actively participate in treatment sessions, be open to trying different approaches, and communicate openly about your experiences.
  • Maintaining Progress:
  • What to Expect: Ongoing care to prevent your pain from returning and to support your long-term wellbeing. This may involve less frequent visits and a focus on self-management strategies.
  • Your Role: Consistently perform any recommended exercises or stretches, make healthy lifestyle choices, and stay connected with Dr. Jones for ongoing support.
  1. Time Frame and Frequency

We’ll work together to gradually reduce the frequency of your visits as your condition improves:

  • Weeks 1-4: 2-3 sessions per week.
  • Weeks 5-12: 1-2 sessions per week.
  • Weeks 13-24: 1 session every other week.
  • After 6 months: Transition to monthly sessions or less, as needed, to maintain progress and prevent flare-ups.
  1. What Happens During a Session?
  • Initial Consultation: This will be a longer appointment (45-60 minutes) to allow for a detailed assessment, discussion of your condition, and initial treatment.
  • Follow-up Treatments: These sessions (15-30 minutes) will focus on providing hands-on care, monitoring your progress, and adjusting your treatment plan as needed.
  1. Your Active Role in Long-Term Wellbeing
  • Partnership: Work collaboratively with Dr. Jones to develop and implement your care plan.
  • Self-Care: Prioritize activities that support your physical and mental health, such as exercise, stress management, and good sleep habits.
  • Ongoing Communication: Stay connected with Dr. Jones to address any concerns and ensure your treatment plan continues to meet your needs.
